New Patient Paperwork

Cooperstown Chiropractic offers our patient form(s) online so they can be completed in the convenience of your own home or office. We do ask you to go ahead and fill out the forms prior to your first consultation appointment. If you are not able to complete the forms online, we will have these forms for you when you arrive. We do ask you to arrive 15-20 minutes early if you need to fill out the forms in our office.
